I, the parent or legal guardian and manager of the players, whose name is listed on the same line with my signature, hereby give my consent for his/her participation in the Richmond Latino United 3v3 Summer Fiesta Tournament held on August 8-9, 2026 and hosted by Richmond Latino United Soccer Club. I recognize the possibility of physical injury associated with soccer, and hereby release, discharge, and otherwise indemnify Richmond Latino United, NCYSA/USYS, US Club Soccer, SAY, AYSO, USSSA, USSDA, AAU their sponsors, the USSF and its affiliated organizations, and the employees and associated personnel of these organizations, including owners of facilities utilized for such tournament, against any claim by or on behalf of the player as a result of the player’s participation in the Richmond Latino United 3v3 Summer Fiesta Tournament, and/or being transported to or from the same, which transportation I hereby authorize. I, the parent or legal guardian of the player, whose name is listed below on the same line with my signature, hereby give my consent to have an athletic trainer, coach, team manager, emergency medical technician, nurse, medical treatment facility, and/or doctor of medicine or dentistry or associated personnel provide the registrant/participant with medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. I hereby authorize emergency transportation of the player to a medical treatment facility should an individual described above consider it to be warranted.
